Disease Prevention and Control

What you need to know to Pre-Register and for your visit to our clinic:

• If you are a NEW client to our health clinic, you can get faster service by pre-registering before your visit to our clinic (at least 12 hours prior to your appointment time)
• A parent or guardian needs to come into the clinic with the child for him/her to receive health care services.
• Important - Please bring the following items with you:
  • For immunization services, bring any/all immunization records you have for each person
  • If a doctor referred you to us for services, please bring any documentation that was given to you to your appointment
  • Picture ID
  • Insurance card(s) such as Third Party, Medicaid, and we take Medicare Part B (only for Flu and Pneumonia shots)
  • Income verification - if you would like to be considered for discounted services bring document(s) such as:
    • Recent paycheck stub
    • Unemployment check stub
    • Current Tax return form

Services can be discounted based on income and family size. You will not be denied services due to inability to pay.
